DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government team on a 12-month assignment with a strong possibility of an extension of up to three years. Hours are full time (Mon - Fri) and will be based in Lismore.
The position of Clinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members and/or their families.
The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at-risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.

The role requires you to:
- Liaise and form partnerships with various internal and external stakeholders
- Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.
- Maintain clinical records, collating and analysing data from client records & preparing reports as required
- Work systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and families
- Participate in community development and service promotion activities
- Contribute to the development and delivery of group programs
- Qualifications as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR
- Qualification as a Social Worker eligible for membership of the Australian Association Social Workers (AASW) and be eligible to apply for Mental Health accreditation.
- Working with Children (WWC) and/or Vulnerable People Checks/Clearanc
- Candidates must have extensive and relevant clinical and case management experience.
- An Australian Citizen and willing to undertake a National Police Clearance.
- An attractive hourly rate of up to $65 per hour + super
- Monday to Friday, 37.5 hours per week
- 12 month contract with a potential of extensions of up to 3 years
