DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients team. This role is full time hours (Mon - Fri) and will be based in Townsville.
The position of Clinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of their vulnerable clients.
Clinical Care Coordinators provide short-term, goal-oriented, clinical case management intervention to clients who present with unmet health and psychosocial needs that impact on their ability to access and/or meaningfully engage in mental health care. With a primary focus on promoting client engagement with mental health care, Clinical Care Coordinators, assist clients in identifying appropriate supports, reduce barriers to access, and address health and psychosocial factors likely to negatively impact on engagement in care, or lead to significant deterioration in mental health if left unaddressed. The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at-risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.

The role requires you to:
- Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.
- Liaise and form partnerships with various internal and external stakeholders
- Maintain clinical records, collating and analysing data from client records & preparing reports as required
- Work systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and families
- Participate in community development and service promotion activities
- Contribute to the development and delivery of group programs
- Degree qualified Registered Mental Health Nurse, Psychologist, Social Worker eligible for MH accreditation or Mental Health Occupational Therapist
- Full registration with AHPRA or eligible for AASW
- Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card)
- An Australian Citizen and willing to undertake a National Police Clearance.
- An attractive hourly rate of $65 per hour + super
- Contract until 30th June 2024 with extensions - this role is considered an ongoing requirement
- Monday to Friday, 37.5 hours per week
