Job description

DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients team on a 12-month assignment. These roles are full time hours (Mon - Fri) and will be based in Winnellie.

The position of Clinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members and/or their families. 
 
Reporting to the Assistant Director, The Clinical Care Coordinator incorporates the development and maintenance of internal relationships within the Department as well as external relationships through the provision of direct evidence informed support to individuals / their families with complex needs.  The role will also involve providing phone or in person assessment, support and linkage to internal and external services to clients presenting with immediate needs.
 
The role requires you to:
  • Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.
  • Reviews of intake risk assessments, safety planning and coordination of timely and appropriate allocation and intervention for clients seeking services.
  • Liaise and form partnerships with internal and external stakeholders.
  • Maintain clinical records, collating and analysing data from client records & preparing reports as required.
  • Work systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force. communities and families.
  • Participate in community development and service promotion activities.
  • Contribute to the development and delivery of group programs.
About you:
  • Degree qualified Registered Mental Health Nurse /  Psychologist, Mental Health Occupational Therapist with full AHPRA registration or
  • Qualification as a Social Worker eligible for membership with AASW and be eligible to apply for Mental Health accreditation
  • Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card)
  • An Australian Citizen and willing to undertake a National Police Clearance.

  • An attractive hourly rate of up to $65 per hour + super 
  • Monday to Friday, 37.5 hours per week
  • Extensive training and ongoing support & development
  • 12 month contract with a potential of extensions of up to 3 years
